# When a form fails, and certificates

Choose what happens when a check fails, and issue a numbered certificate with an expiry date when a form passes.

Open the form in **Settings → Form templates** and select the **Settings** tab.

## When the form fails

A form fails when a pass / fail question is answered **Fail**, or a reading is outside a range set to fail. When the engineer finishes the visit, Sustainable ERP can:

- **Tell the office**: an alert in Sustainable ERP with the job. On by default.
- **Mark the asset faulty**: the asset the form was for shows as faulty, with the failed check as the reason.
- **Raise a remedial job**: a new job linked to this one, ready to book.

On planned maintenance jobs, a failed check also raises a remedial job when your company has that switched on for PPM.

## Certificates

Tick **Issue a certificate when this form passes**, then set:

- **Certificate title**, like "Gas safety record".
- **Valid for (months)**. Leave it blank if it doesn't expire.

When the engineer finishes a visit, each form that passed gets a certificate with its own number, like CERT-000012. The certificate PDF goes on the job's files, where the customer can see it. Failed forms don't get a certificate.

Select **Save settings**. The change applies to visits finished from now on.
